Answers:    Do you need a spinal tap ? Yes.
Can they diagnose MS from a single noncontrast MRI ? NO

Multiple sclerosis (MS) is often exceptionally difficult to diagnose. There is no single test for this because every abnormality in a test contained by a patient with MS can be created by some other disease. This includes the spinal tap. It is diagnosed base on the symptoms with the tests supporting the diagnosis. It is hard to diagnose from symptoms alone because every symptom of MS can be cause by another disease. The doctors need every piece of information to make the diagnosis as soon as possible. There are patients who carry a diagnosis of possible or probable MS for years up to that time it is clear.

Is it important to know now ? Yes

Most people believe of the disease as occurring in fits and starts with attacks and remissions. Research shows that the disease is slowly progressing even when the person feel no symptoms. The new drugs cannot cure the disease but they can slow or even stop the progress. For a patient to do the best possible they need to be started at the inauguration because the damage caused by the disease cannot really heal. All destroy is therefore cumulative. The drugs are expensive and do have potential side effects. Your doctor would not want you to use them if you did not have MS but would not want you to continue if you did have it.
I am so sorry. I have MS, too. If the doctor wants a spinal stroke, let him do it. It isn't that bad, just lay really still. The plaque can be detected on the MRI near contrast, but the spinal tap gives even more reason to prove you own MS. When you file for SSDI, which you can with MS, they will have positive proof. Good luck to you. Make sure you capture on Avonex or some other MS drug as soon as possible. MS is a diagnosis made by excluding everything else that can give you the scarring seen on the MRI.
