How credible is germs to travel from the cerebrial spinal fluid (csf) into the bloodstream?

A blood culture determined no bacterial infection, but the doc wants me to schedule a neurological appointment for a lumbar puncture. This stems from brain surgery in 2005. Wouldn't germs have entered the bloodstream by now even if it be initially confined to the csf?
Not necessarily. Lots of organisms, bacteria, even some medications don't cross the blood-brain barrier.
