Ms. Johnson, an automobile whiplash victim, has been suffering spinal shock, but is looking forward to complete recouping. One night on evening rounds, her nurse discovered her in a fetal position, her body drenched with sweat. She be incontinent of feces and urine and her blood pressure was dangerously high. Spinal shock is characterized by flaccid paralysis (loss of bladder tone) below the level of the injury, and loss of spinal cord reflexes including bladder and bowel sphincter control. Spinal shock repeatedly coincides with neurogenic shock, which is evidenced by loss of autonomic control (bradycardia, vasodilation, hypotension). Spinal shock can persist for several weeks, and is associated with an inability to retain urine or bowel contents. Although urinary incontinence may be present, bladder empty is
also dysfunctional, making some form of catheterization mandatory.
